Covid 19 Contact Tracer | Department of Public Health | November 2021 – June 2022

• Making follow up calls to patients after initial call by EPI/Contact Investigator

• Documenting the names and contact information of all individuals who have had contact with a COVID-19 patient.

• Tracing and notifying all contacts who have potentially interacted with an infected individual.
• Provide contacts with approved information about isolation and quarantine procedures, and if appropriate, refer them to testing according to protocol and/or to a COVID-19 Community Support Specialist for social resources.
• Follow a set script to inform contacts about the importance of isolation or quarantine and what to do if symptoms are present or develop.

• Maintaining appropriate discretion with the information.

• Tracking and documenting all health data (using an IHS provided software)

Breastfeeding Peer Counselor | Baldwin County Health Department | October 2019– October 2021

•         Gives basic breastfeeding information and support to new mothers, including telling them about the benefits of breastfeeding, overcoming common barriers, and getting a good start with breastfeeding.

•         Helps mothers prevent and handle common breastfeeding concerns.

•         Counsels WIC pregnant and breastfeeding mothers by telephone, text messaging, home visits, and/or hospital visits at scheduled intervals determined by the local WIC Program.

•         Counsel women in the WIC clinic.

•         Attends and assists with prenatal classes and breastfeeding support groups.

•         Attends WIC staff meetings and breastfeeding conferences/workshops as appropriate.

•         Manages a caseload of WIC mothers and makes routine periodic contacts with all mothers assigned.

•         Refers mothers, according to clinic-established protocols, to: - WIC nutritionist or breastfeeding lead - Lactation consultant - Mother’s physician or nurse - Public health programs in the community - Social service agencies.

•         May assist WIC staff in promoting breastfeeding peer counseling through special projects and duties as assigned.

•         Other related work as required.
